New Patient Registration
We welcome patients in the villages of Hethersett, Great Melton, Little Melton, Ketteringham, Bawburgh, Wramplingham, Barford and the surrounding areas. A map is available below. If you are unsure whether you live in our practice area please pop in or telephone us for more information.
We ask that new patients fill in a standard NHS registration form as well as our practice patient questionnaire. These are available for downloading from the above Registering Online tab. Please bring the completed forms, one for each family member, to the surgery. Hethersett is a family practice and we would not normally accept just one member of a household.
In accordance with a Government directive, all patients have been allocated a named doctor at the surgery they are registered with. However, you can still book an appointment to see whichever doctor you prefer, or whoever is available at a time most convenient to you. If you would like to know who your named doctor is, please ask at reception when you next visit.
New Patient Checks
Every patient, over 16, registering with one of our surgeries will be offered a new patient registration medical. The appointment gives you a chance to meet us, have your health checked and bring to light any on-going medical problems or important past medical history before we receive your medical records from your previous surgery. Your appointment will be with either a doctor or practice nurse and you will be asked to bring with you a urine sample for analysis. The new patient registration medical also allows us to provide repeat medication from your previous surgery.
PLEASE NOTE: If you are on regular repeat medication you will need to see one of our doctors BEFORE a prescription can be issued.
